Continuing Care Aid (CCA)

Job Overview

The CCA will be responsible for the care pertaining to each family from the time of admission to the discharge. The CCA documentation will work closely with each family and support all  aspects of the care plan and the recovery journey and will assist the care team with identifying  and addressing risk factors associated with each family. The CCA is responsible for ensuring that  all duties are completed to the standards of the organization. The CCA will report to the Shift Supervisor as the primary Supervisor. 

Key Responsibilities: 

• Reports to the Shift Supervisor and Registered Nurse for day-to-day operations • Assist the Shift Supervisor, Registered Nurse, and therapists in ensuring the daily  treatment schedule is adhered to by clients 

• Coaching parents in personal care related to their children including: grooming, bathing,  personal hygiene, toileting, wound care, and nutrition. 

• Assist Registered Nurse or Shift Supervisor with and support residents with medication  adherence.  

• Work with residents on goals and care plans aimed at recovery and management of their  substance use disorders  

• Build rapport with residents and provide therapeutic interactions for multiple issues  affecting their ability to cope and manage in the community.  

• Advocate on behalf of residents for additional service and resources, as required. • Establish and maintain current, accurate, confidential charting and documentation for  each resident. 

• Assist with transition plans into the community set by the client and care team. • Follow medical instructions carefully and consistently. 

• Manage any unanticipated events or unstable situations. 

• Support other departments within The Carter House with additional duties such as  cleaning and maintenance.

Educational Requirements and Qualifications: 

• Certificate in Continuing Care Assistant or equivalent certification or diploma  • Must retain active CPR and First Aid certification. 

• Must retain Food Safety Certificate 

• Experience in a healthcare setting, such as a mental health facility, addiction treatment  centre, nursing home, or community health organization. 

• Experience in direct patient, client and family care roles. 

• Understanding of charting and documentation standards in a healthcare environment.

Hours of Work: 8-hour shifts on Days, evenings, weekends 

Expected Salary Range: $20-$24 hourly
